One Pain, Two Perspectives - Understanding Neck Pain

Neck pain affects people of all ages, from mild muscle stiffness to severe pain with upper limb numbness or weakness. Modern habits like prolonged screen use have increased its prevalence. Whether it is a stiff neck from sleeping awkwardly or a slow-building ache from years of desk work, understanding the cause is the first step toward recovery.

What Causes Neck Pain?

Neck pain can arise from how we move, rest, and even how we feel emotionally. Both Traditional Chinese Medicine (TCM) and Western medicine offer useful frameworks for understanding it.

The TCM View: 6 Types of Neck Pain

In TCM, neck pain falls under "Bi Syndrome" (Painful Obstruction) - describing a neck that is either blocked (sharp pain) or lacking nourishment (dull ache). Six common patterns are recognised:

  • The “Air-Con” Effect: Sudden stiffness and limited movement worsened by cold or wind, relieved with a warm compress.
  • The “Desk Worker” Knot: Sharp, fixed pain in one specific spot with hard, tender muscle knots, caused by prolonged desk work or an old injury.
  • Age-Related Aches: Chronic, dull soreness are common in older adults, often paired with weak knees or a tired lower back.
  • The “Heavy Shoulder” Burden: Heaviness in the head, neck, and shoulders, sometimes with brain fog or dizziness.
  • Stress Tightness: Neck and shoulders lock up during anxiety, stress, or high blood pressure. Often triggers tension headaches.
  • Frail or Elderly Weakness: An integrated approach gives you the best of both worlds.

The Western Medicine View

Western medicine identifies neck pain through anatomy and biomechanics:

  • Posture and Ergonomics: "Tech Neck" – Pain from poor posture, hunching over laptops, or looking down at a phone.
  • Muscle Strains: From awkward sleeping positions, sudden twists or whiplash from accident(s).
  • Degeneration / Wear and Tear: Slipped discs, bone spurs, or worn joints pressing on a nerve.
  • Stress: Muscle tension across the back of the neck radiating to the head or shoulders.

⚠ Red FlagIf your neck pain shoots down your arm, or causes numbness or weakness in your fingers, see a doctor early.

Treatment Options

Most neck pain responds well to treatment and often benefits from combining Eastern and Western methods.

TCM Modalities

  • Needle Acupuncture (with or without electrical stimulation): Targets affected meridians to relieve pain and restore balance.
  • ElectroacupunctureMuscle tension across the back of the neck radiating to the head or shoulders.
  • Laser AcupunctureA needle-free alternative.
  • Cupping TherapyImproves blood circulation and reduces. Frequently paired with acupuncture.
  • Auricular Therapy (Ear Acupuncture): Targets ear points including Kidney (肾), Spleen (脾), Liver (肝), Shenmen (神门), Sympathetic (交感), and the Neck zone — to treat corresponding areas of the body.

Western Medicine Methods

  • Posture correction and ergonomic modificationsReduces ongoing muscular tension through patient education.
  • Physiotherapy: Stretching, muscle strengthening and conditioning, heat therapy, and posture correction.
  • Oral medications: Anti-inflammatories, muscle relaxants, and pain relievers.
  • Injections: Radiologically guided, for more severe or persistent pain.
  • Surgical intervention: Reserved for severe nerve compression, spinal instability, or neurological deficits.
