Our aim is that each patient is assigned a dedicated Primary Care physician, to ensure seamless patient care integration during transition between community and hospital settings.

By ensuring every patient has a designated Primary Care physician, we foster continuity of care and enhance patient outcomes across different healthcare environments.

As part of the national Healthier SG plan (Healthier SG), we collaborate with GPs to assist residents in achieving their health goals and connect them with community resources such as programmes, activities and services.
