The following resources were developed to help you prepare for your hospital stay, to inform you of all the services and facilities we offer and to make your stay with us as smooth and comfortable as possible.
Patients scheduled for elective surgery will be referred to our Pre-Admission Counselling and Evaluation (PACE) services at Basement 2, TTSH Medical Centre.
You will be given all the information you need regarding the proposed procedure or surgery and how best to prepare yourself.
Pre-operative evaluation and tests will be conducted to ensure that you are fit for the proposed surgery. Financial counselling and information on your estimated bill size will also be provided.

Tan Tock Seng Hospital is committed to providing a safe and accessible environment for all patients, visitors, and staff. To support this, the use of Personal Mobility Aids within hospital premises is regulated.
What are Personal Mobility Aids (PMAs)?
PMAs refer to wheelchairs, motorised wheelchairs, and mobility scooters (3 or more wheels with a seat) designed for individuals who are unable to walk or have walking difficulties.

What's NOT Allowed Indoors
"Personal Mobility Devices (PMDs) and Power-Assisted Bicycles (PABs) are strictly prohibited indoors. This includes kick-scooters, electric scooters, hoverboards, unicycles, motorised bicycles, and large PMA lookalikes (e.g. mobility scooters or devices designed to carry multiple riders). Users must dismount and walk when inside hospital buildings.

Where can I use my PMA?
Outpatient Areas - PMAs are allowed for use in outpatient clinic areas. Please show consideration for others when using your PMA around the hospital.
Inpatient Areas - PMAs must be parked at designated ward-level parking bays and are not allowed at patient bedsides.
Emergency Department - PMAs are allowed in the Emergency Department.
If you are scheduled for admission, you are strongly encouraged to arrange for your PMA to be brought home. If unable to do so, PMAs must be parked at designated ward-level parking bays.

Speed limit: Please ride at walking speed of 5 km/hr for the safety of all patients, visitors and staff
Parking: Please park your PMA at designated ward-level parking bays
For Patients Only:
Charging: Charging of PMAs is allowed only for patients upon discharge with a 30-minute limit as a safety precaution. Charging must be done at designated charging areas around the hospital and is not permitted inside wards or at patient bedsides

For Visitors:
Charging of PMAs is not allowed for visitors as a safety precaution

- Patients may be assigned to a bed located in HealthCity Novena such as TTSH wards at Renci, Community Hospital, National Centre for Infectious Diseases, or Integrated Care Hub. Dedicated care will be provided by TTSH Doctors and nurses.

How to apply for medical information?
What is required when applying for medical information?
For us to proceed with any application, the hospital needs the patient's authorisation before any medical information can be provided.
- For patients below 21 years of age, consent of the parent or legal guardian is required.
- For deceased or mentally incompetent patients, consent from the appointed representative of the estate is required. Where applicable, the Consent and Declaration for Release Of Medical Information for patient with Mentally Incapacitated or Deceased Patient (Form B) (PDF, 570 kb) must be filled up. A copy of the death certificate and other supporting documents (e.g. marriage certificate, birth certificate, letters of administration) are required as proof of the representative's relationship to the patient.
- Interested third parties can write in together with patient's consent, permitting the hospital to release medical information for a stated purpose.
- For foreign workers that have returned to their native country, employers can apply for these foreign workers' medical information by completing a Letter of Indemnity and also providing document proof of cancellation of work permit for the foreign worker.

Ordinary Medical Report
This is a factual report of the patient's medical condition. A doctor will write the medical report based on patient's medical information available. (Insurance form or type-written report)
Work Injury Compensation (WIC) Initial Assessment Report
This is for the purpose of determining work-related injuries and the degree and period of disability results of a work injury. Requester to provide MOM's WIC initial assessment form or MOM request letter for the application. May involve a review at the Specialist Outpatient Clinic.
Specialist Medical Report
This report is prepared by the patient's specialist in response to requests that require a professional opinion with regards to the patient's prognosis and disabilities. It is based on an actual assessment of the patient and may involve a review at the Specialist Outpatient Clinic. (Insurance form or type-written report)
Work Injury Compensation Objection Assessment Report
This is for the purpose of re-determining work-related injuries and the degree and period of disability when patient objects to the results of the WIC initial assessment report raised. The WIC objection report is done by two doctors who are consultants and above. We accept WIC objection report request for patients who are assessed by doctors of other hospital for the initial assessment report.
Specialist Medical Report (Simple - Psychiatric)
This report is prepared by the assigned psychiatrist in response to requests that require a professional opinion with regards to the patient's assessment or condition. The requirement for further assessment for purpose of preparing the report is subject to review by the assigned/treating psychiatrist in the Specialist Outpatient Clinic.
Specialist Medical Report (Complex - Psychiatric) / Court Appointment for Deputy (CAD) Report
This report is prepared by the assigned psychiatrist in response to requests that require detailed professional opinions not covered by a simple report, with regards to the patient's assessment or condition, which may extend to domains of prognosis and disability. The requirement for further assessment for purpose of preparing the report is subject to review by the assigned/treating psychiatrist in the Specialist Outpatient Clinic.
Medical report for Court Appointed Deputy Application (Form 61), patient has to be examined by a psychiatrist. At present, a lawyer is required to initiate the request and we are unable to provide reports for applicants acting in person/via iFAMS.
Lasting Power of Attorney Certificate Issuance
The LPA Certificate may be issued by accredited medical practitioners, lawyers and psychiatrists. As charges vary according to complexity of the cases, members of the public are advised to check on their requirements with the Office of the Public Guardian before selecting a certificate issuer.
Ordinary/ Factual Medical Report (Psychiatric)
This is a factual report of the patient's medical condition based on clinical information that is already available. No assessment will be required for preparation of the report. In the event that a specific psychiatric opinion is required, the report will be considered as Specialist Medical Report (Simple – Psychiatric) or (Complex – Psychiatric).
Second Opinion Reports
This report is written for people who are not patients of TTSH but seek a professional opinion from our Specialists. Its format and scope is the same as the Specialist Medical report. It is based on the assessment of the patient and involves a review at the Specialist Outpatient Clinic.
Duplicate Copy of Investigation Results
Selective duplicate copies of investigation results such as blood test results, lab results, investigation reports (ECG, Electrophysiology, EMGS), X-ray and other radiological reports can be made available.
*Investigation results may be available for free on the NHG Health App. Please check the app before submitting an application to the Medical Records Office.
Certified True Copy of Medical Report or Reprint of Medical Certificates Issued
A duplicate copy of the medical report that was previously applied before or a reprint of medical certificate previously issued.

When will my medical information be ready?
As a general guide, the time frame for completion of medical reports is between 4 to 6 weeks.
It may take longer if:
- Patient has upcoming clinic appointments
- Patient has been hospitalised
- Multiple reports are required from several clinical departments
- Doctor is away
Specialist Medical Reports and Workmen Compensation cases require more processing time as a review at the Specialist Outpatient Clinic may be required after the patient has been discharged or given open date from clinic review.
How can I receive my medical information?
To obtain medical reports, insurance forms, or copies of medical records, please submit your request via FormSG. All medical information will be released in digital format only and sent to the email address your provide in your application.

Overstaying
Social over-stayers are patients who have been certified to be medically fit for discharge from acute care and assessed to have feasible discharge options, but continue to stay in acute hospitals for prolonged periods. To ensure that hospital beds are available to patients requiring acute care, Government subsidy, MediShield Life or Medifund will not be extended further after the patient is certified to be medically fit for discharge from the hospital, if the patient decides to remain in the hospital. Integrated Shield Plans (IPs) and Civil Service Medical Benefits will similarly not be extended further. This applies to all inpatient ward classes.
