Your Hospital Discharge
The following resources were developed to help you prepare for your hospital discharge, to inform you of all the services and facilities we offer after your discharge from the hospital.
Notice
Approach your care team to plan early and discuss post-discharge matters.
Thank you for giving us the opportunity to manage your care. You have made significant progress of your recovery and it is time to continue the healing journey at home.
Continuing your recovery at home
The appropriate post-discharge care services can enable patients to meet their needs in the community.
These services include Centre-based, Home-based, Institution-based care or other support services.

While awaiting longer term plans
You and your family can plan for an interim care arrangement at home. If eligible, you may also be discharged to interim care facilities while waiting for your final placement.
Overstaying
Patients who are identified as Medically Fit for Discharge from Hospital and refuse discharge either to an interim placement or final placement may be deemed as a Social Overstayer. In line with the Ministry of Health’s Social Overstayer Policy, any extended hospital stay will result in:
- Withdrawal of government subsidies
- Inability to utilise MediShield Life, Integrated Shield Plans, Medifund assistance, Private Medical Insurance, Civil Service Benefits, non-immediate family members’ MediSave and the Public Assistance Scheme for public hospital bills

TTSH Mobile Inpatient Care at Home (TTSH@Home) caters to the preference of patients who seek comfort and familiarity when receiving care. As an alternative to conventional hospital stays, our clinical services are delivered to medically suitable patients in their residences. Our dedicated team of doctors, nurses, and allied health professionals ensures comprehensive care while monitoring patients remotely.
With TTSH@Home, we aim to redefine the healthcare experience and empower our patients.
Services offered
During home visits, our clinical team provides tailored care to meet your needs. Our range of services encompasses both remote and in-person modalities which include but not limited to:
- Remote care monitoring
- 24-hour call concierge to attend to your non-emergency medical queries
- Home visits by our clinical team
- Support the administration of intravenours infusion or injections
- Sample collections for lab investigations
- Bladder and wound management
- Caregiver training and patient education
- Medication delivery
- Return to the hospital for scans and day procedures
Conditions suitable for TTSH@Home
Patients who are suitable to continue receiving care at home may have cellulitis, urinary tract infections, pneumonia, gastroenteritis, dengue fever, rhabdomyolysis, influenza, and viral fever. Other conditions, such as COVID-19 infections in patients with weakened immune systems, as well as post-operative care for selected patients following surgery, are also considered.

How to get a referral
Getting a referral
Patient's doctor will raise a referral. Patient can also inform their doctor on their interest in TTSH@Home.
Assessing your suitability
Our MIC clinical team will assess patient's suitability for the service. Once patient consented, patient will be enrolled into TTSH@Home.
Preparing for TTSH@Home
Our MIC staff will explain the service and charges. They will also educate patient on the aspects of remote care monitoring and what to do during a medical emergency.
Leaving the hospital
You will receive a Vital Sign Monitoring kit (VSM), enrolment file, and medications to be taken or administered at home.
Care in TTSH@Home
You are now on your way home to conitnue your treatment and recovery journey.
How much will it cost?
Patients are eligible for government subsidies and they can tap on MediShield Life & Medisave in line with the existing Acute Inpatient Financing Framework by the Ministry of Health (MOH). In addition, patients can also utilize their Civil Service Card and Integrated Shield Plans*. Patient will be counselled on the charges prior enrolment to TTSH@Home service.
*Please check with respective Integrated Shield Plans provider on the details of your coverage.
