We are honoured to be recognised as an Age-Friendly Health Systems Participant by the Institute for Healthcare Improvement (IHI). With this, we join an international group of more than 5,200 health systems working to tailor care to patients’ goals and preferences and deliver consistently high-quality care.

This recognition affirms our ongoing work to care for older patients through the 4Ms: What Matters, Medication, Mentation and Mobility.
 

Across TTSH, this shows up in practical ways:

• Personalised headboards that help teams understand what matters to patients
• Medication reviews that support safer prescribing
• Delirium care to detect confusion early
• Mobility protocols that help older patients keep moving safely

It also builds on years of geriatric care work, including our Geriatric Emergency Medicine nurses, who assess and support older patients with age-related needs at the Emergency Department, and our 2023 NICHE Exemplar recognition, the highest level awarded by an international programme recognising excellence in nursing care for older adults.
