Patient Reported Outcome Measures (PROMs)

Listening to What Matters Most

Patient-Reported Outcome Measures (PROMs) are questionnaires that allow you to share how you feel, function, and cope in your daily life. By capturing information directly from you, PROMs bring your voice into care and help healthcare teams better understand your experiences, needs and priorities.

Why are PROMs important?

PROMs help us understand aspects of health that may not be visible through tests or clinical assessments alone.

By sharing your experiences, symptoms and concerns, your healthcare team can better understand:

  • How your condition affects your daily life
  • What symptoms or challenges matter most to you
  • Whether your current treatment plan is helping you achieve your health goals
  • How your health and wellbeing change over time

This helps ensure that care is tailored to your needs, preferences, and priorities.

How to participate?

  • Depending on your condition, your healthcare team may recommend specific PROMs questionnaires and advise you on how often they should be completed.
  • PROMs can be completed conveniently through NHG Health App before your clinic visit. You may also receive an invitation to complete them between follow-up visits.
  • The information you share will be reviewed by your healthcare team and used to support discussions, care planning, and treatment decisions.
