Health Coaching at the Community Health Post began in 2016 to help residents who are at risk of medical conditions better manage their health. The Health Coaches provide coaching on lifestyle modification and facilitate intervention programmes. To further empower the community to better manage their health and lead healthier lifestyles, a network of Peer Support Leaders (PSLs) was established. These PSLs are equipped with the skills, knowledge and confidence to function as extenders of the Health Coaches in the community. They are not only empowered to look after their peers but are also given the opportunity to lead and co-create programmes and activities for their peers and residents in the community.

With the increasing number of Diabetes Mellitus (DM) patients and the rising disease burden, a risk stratification-based DM management programme, Integrated Diabetes Care Programme (IDCP), was conceptualised to improve the quality of patient care, patient safety and long-term diabetes outcomes for inpatients with DM. The multidisciplinary team, comprising endocrinologists, pharmacists and diabetic nurse clinicians, proactively reviews and manage high and intermediate risk patients with DM. The team also uses a newly developed IT system (System for Proactive Assessment of RisK – SPARK) which utilises data from electronic medical records to stratify patients with DM into high, intermediate and low risk categories.

Project Care is a palliative care initiative that aims to improve the quality of life and care of nursing home residents in the seven nursing homes in Central Singapore. Led by a multidisciplinary team comprising doctors, nurses, medical social workers and administrative team, the programme enables residents to have their end-of-life care choices respected and allows them the dignity of spending their final days in their preferred environment in comfort and receiving their preferred treatment, thus reducing unnecessary hospital transfers. The team has also empowered the nursing homes so that they are able to conduct Advance Care Plan discussions independently, taking into account the residents’ treatment preference and preferred place of death.

The Clinical HIV Programme was established as a multidisciplinary team to deliver seamless, holistic clinical and psychosocial care to people living with HIV/AIDS (PLHIV) in Singapore, and provides care for the largest cohort of PLHIV in the country. Apart from providing specialist HIV medical and nursing care, the Clinical Programme also comprises the Positive Prevention Programme, Defaulter Programme, Durg Adherence Programme and the Data Management Programme.

To transform care provision from fragmented care to patient-centred care that is coordinated and aligned across multiple care providers, a multidisciplinary team was set up to manage patients with complex chronic diseases. A key enabler for the care management of such patients is the assignment of a Primary Coordinating Doctor, who provides oversight of care goals through active medical care plan reconciliation prior to and during outpatient clinic reviews.

TTSH Coordinating Advisory Care Team (CoACT) is a clinical advisory group that enables bidirectional communication, collaboration and cooperation between the hospital specialists and primary care providers. TTSH provides a single point of contact from the various respective clinical departments to support primary care should they need advice on sub-specialty-related problems for patients under their care. Serving as a continuous liaison between primary care physicians and hospital specialists, CoACT ensures smooth transition for patient care into the community with both administrative and clinical support from specialists in the hospital.

Well recognised and sought after for his expertise in anaesthesia, general ICU and neuro-intensive care and performing percutaneous tracheostomy, A/Prof Kwek is an all-rounded clinician-leader who has been contributing consistently in areas of clinical work, education and service development. During his long career with TTSH, he has been appointed to many leadership positions and held numerous administrative portfolios on top of his clinical responsibilities.

A/Prof Jackie has been the Head of Department of General Medicine since 2008. Heading the largest department in the Division of Medicine and TTSH, A/Prof Jackie has successfully handled a wide range of clinical, educational and administrative portfolio. As a Senior Consultant, she is one of the few senior “home-grown” internal medicine (IM) physicians with well-rounded IM training and has established herself in Singapore and the region in the subspecialty of obstetric medicine.

A/Prof Ong has and is still serving TTSH for over 20 years in his capacity as Emeritus Consultant and previous Senior Consultant at the Psychiatry department. He was the first Head of Department when Psychiatry, then known as Psychological Medicine, was first founded in TTSH in 1986. A/Prof Ong shaped the culture and went on to achieve many things for the good of the patients and the psychiatry practice in Singapore.

As Executive Director of NCID, Prof Leo has led her team through multiple outbreaks in Singapore. These include the Nipah outbreak in 1999, SARS in 2003, pandemic influenza in 2009, Zika outbreak in 2016, Monkeypox in 2019 and most recently, the COVID-19 outbreak.

As Divisional Chairman (Medicine) since 2018, A/Prof Thong has maintained very high standards of clinical care, ensuring constant checks and audits of work and setting principles that can be applied across the Division of Medicine. He is heavily involved in numerous cluster- and national-level committees and workgroups. Despite his busy schedule and responsibilities, he is still able to contribute greatly to his clinical field of Rheumatology, Clinical Immunology/Allergy and Internal Medicine.

As Divisional Chairman of Ambulatory & Diagnostic Medicine, A/Prof Chong leads various groups of clinical and support service departments and has been actively providing guidance and mentoring them in various projects and initiatives. Through the years, she has maintained her steady leadership and has displayed outstanding grasps of key issues in areas under her care. With years of experience in Radiology, specializing in Breast Imaging, she is widely recognized as an expert at the national level.

A/Prof Chong is a well-known urologist with excellent skills. Upon returning to Singapore in 2008 after his fellowship training, A/Prof Chong was appointed as faculty and Consultant Urologist at TTSH, and helped develop a range of new surgical capabilities at the hospital. This included the development of the hospital’s inaugural robotic prostatectomy programme. As Divisional Chairman of surgery, A/Prof Chong has been continuously developing a strong culture of belonging, collaboration and innovation while ensuring values of professionalism and clinical excellence within the division.

A surgeon’s journey starts by learning from seniors, colleagues, and patients. I feel fulfilled in giving of myself to generations of urologists so that they in turn can benefit their patients.

A/Prof Wong is a respected leader who possesses both good technical competency as well as great soft skills. She has contributed greatly to the many strategic thrusts in various clinical areas including the Intensive Care Unit. As a senior clinician, A/Prof Wong also built strong networks with other departments and laid the foundation for the close working relationships within TTSH. Described as a great coach, she takes the time and effort to provide support and encouragement to people around her. As one senior colleague puts it, "Every department needs a Mary. She is its heart."

Ms Loh started her career in healthcare industry with TTSH in 1993 as an Executive in the Medical Affairs department and rotated through multiple administration and operations portfolios. Throughout her postings in TTSH, she has been involved in many pioneering projects such as the digitization of medical records. She was then seconded to the Home Nursing Foundation in 2009 and was a pioneer in connecting the acute hospital with providers in the ILTC sector. In 2011, she became the CEO of Ren Ci Hospital and strengthened its clinical capabilities so that it is now a key and trusted collaborator for TTSH in its management of complex and long-term care patients. Since her return to TTSH in 2018, she has been engaging partners and creating a community provider for Central Health, so as to facilitate the co-creation of programmes and build multi-lateral partnerships in the community.

A/Prof Chew has shown great skills in applying and undertaking difficult tasks as the Endocrinology leader and Assistant Chairman Medical Board (Medical Manpower Development), and in overall leadership roles at institutional, cluster and national levels. He provides clinical expertise and insight in developing Endocrinology-related clinical services not only in the hospital but extending it to other healthcare partners and the community. As an educator, besides imparting knowledge, he mentors and is a role model for younger clinicians to emulate.

Dr Wee began his career with TTSH Rehab Centre and has since played an instrumental role in the development of allied health service excellence in Rehabilitation. Through his years there, he engaged and led teams of rehab therapists, physiotherapists and occupational therapists, in co-creating the vision of a centre of excellence in clinical, education and research. One of the early pioneers, he steered the development of vestibular rehabilitation and in more recent years, technology and robotics in healthcare. His passion in clinical, research and education has also seen him take up roles and positions in the Rehabilitation Research Institute of Singapore (RRIS) and Singapore Institute of Technology (SIT).

A veteran in Infection Control and having learnt many lessons with various outbreaks, Ms Poh co-leads the Department of Infection Prevention and Control to enhance infection control practices particularly in the area of environmental decontamination through use of technology and innovation.

A/Prof Koh has consistently been a leader who is able to appreciate the big picture and adapt to the rapid changes facing the hospital and entire healthcare industry. He has demonstrated an exceptional willingness to collaborate with other clinical specialties and with hospice care organisations. A/Prof Koh continues to develop the hospital’s end-of-life care agenda and clinical framework, and oversees the development of home palliative care for end-organ failure patients. In spite of his heavy commitments as a clinician leader, A/Prof Koh continues to nurture and encourage an academic culture in his department.

Dr Shawn is widely recognized for his expertise in the fields of Microbiology and Infectious Diseases. Since helming the role of Clinical Director of NCID in 2020, Dr Shawn has played an instrumental role in overseeing the clinical and outbreak preparedness activities in NCID and played a major role in ensuring a concerted campus-wide outbreak management response.

Dr Soon leads a team of 300 nursing staff in NCID wards, Clinic J and Clinic K. Apart from providing leadership direction on clinical care at NCID, she also leads the nursing team in outbreak preparedness and management. Under her leadership, the NCID nursing team has grown and demonstrated high nursing competency. Despite her heavy responsibilities at NCID, she continues to be involved in Infectious Diseases research and has co-authored numerous publications.

When Ms Lynn took up the leadership position in respiratory therapy, the department was faced with challenges of manpower resourcing and development of service standards and staff development. She worked closely with stakeholders to resolve the challenges and through the process, brought the entire team closer together. Strong in clinical skills and knowledge, Ms Lynn has been committed to deliver effective patient care while actively shaping the education culture within the department. As Vice President of the Respiratory Therapists Association, she has worked with the Ministry of Health and leaders of other institutions to develop Entrustable Professional Activities, which are competencies required of therapists in their work.

Dr How is highly regarded and recognised as a leading expert in Enhanced Recovery After Surgery (ERAS) in the region. He played a pivotal role in establishing the ERAS programme in TTSH, which is one of the first two Centres of Excellence recognised in Asia by the International ERAS Society. He and his team have achieved good outcomes for patients in ERAS programme and enabled patients undergoing surgery to have a safer and shorter stay in the hospital. As Clinical Director of the Perioperative Recovery Office, Dr How provides leadership and direction to his multidisciplinary team to ensure sustainability and scalability of best perioperative practices and results.

Ms Ng has made remarkable contributions to the organization in many roles but one of the most pivotal is that of the main Lead in the secretariat to the JCI Oversight Committee for the 3-yearly JCI re-accreditation surveys spanning 9 years since 2008. She played a principle role in the success of the hospital’s many re-accreditation exercises, not only displaying her leadership but also her personal coaching and mentorship skills to her fellow colleagues and support teams.

Ms Ang is an outstanding nurse who has undertaken multiple challenges and roles over the years. As a Pioneer, she supported and was instrumental in developing the concept of Quality Advocates and facilitated numerous sessions of the My Care programme. As a Shared Governance Council model member, she has demonstrated collective leadership to support nurses in defining their practices and improve quality and patient satisfaction through a collaborative decision-making process.

Dr Quek demonstrates good leadership as a member of the TTSH Infectious Disease Outbreak Clinical Workgroup. He oversees the setting up of hospital-level outbreak medical teams. Even when he was a relatively young Associate Consultant, Dr Quek had proven to have insights and shown potential in taking up a leadership role in both administrative as well as education portfolios. He is ever ready to take up challenges and has always worked well with fellow colleagues.

Ms Poh joined TTSH as a Management Associate (MA) in 2010 and subsequently enrolled into the Management Executive (ME) programme and Management Fellow programme. Given the numerous learning opportunities through the Management Development programme in TTSH, Ms Poh returns the support by actively grooming the new batches of Management Interns, MAs and MEs. She has driven many hospital-level projects, such as the set-up and operationalization of NCID.
