Online appointment booking can be done via NHG Health App (Downloadable from Apple store or Google Play Store). You will need to login via Singpass to book appointments.

The NHG Health App is a mobile app brought to you by NHG Health as a digital tool providing various services, educational materials and clinical programmes to support a healthier lifestyle for you and your loved ones.

Upon sign-in using your valid SingPass, you will be able to access a range of medical information (e.g. Appointment Management, Registration, View Medication Prescription, Bill and Payment) and key health records (e.g. Laboratory Test Results, Medical Certificate).

If you are new to NHG Polyclinics, please call our Contact Centre at 6355 3000. After having your key information registered in our system, you may book appointments online or reschedule/cancel your existing appointment via the NHG Health App.

Patients currently can make appointments for services such as chronic disease management under a family physician, childhood immunisation, travellers’ clinic, allied health, women’s health and dental.​​

Patients who are in the general pool will not be able to choose to be seen by a particular doctor as we allocate the next available doctor to you.

However, if you choose to be cared for by a family physician, the same family physician will attend to you at each clinic, as far as possible.

Please note that the family physician rates are different from the usual consultation rates.​​​

Online booking is for same day appointment meant primarily for acute conditions like cough, cold, flu, fever.

You are strongly encouraged to obtain your next chronic consultation and/or other required appointment(s), as ordered by the doctor, at our self-service kiosk right after making your bill payment during your regular visit.

Thereafter, you will be able to reschedule or cancel your follow-up appointments, should you choose to do so.

​If you require a vaccination, please note that all vaccinations are strictly on an appointment basis. To book an appointment, please call our Contact Centre at 6355 3000.​​​

Your parents would need to apply for their SingPass account. With that, you can assist them to grant you and/or your sibling access as their caregiver to manage on their behalf.

[For dependent] To add a caregiver, please refer to the following steps:

1. On the home screen, tap on "Dependent/Caregiver".
2. Tap on "My Caregivers".
3. Tap on "CAREGIVER."
4. Input the caregiver's "NRIC" and tap on "Verify".
5. Input the caregiver's "Display Name" and "Email".
6. Select the Caregiver's access to your health records and tap on "NEXT".
7. Input your mobile number and tap on "Proceed"
8. Upon completion, you will see a "Successfully Added" screen.

Alternatively, your parents may call our Contact Centre at 6355 3000 to book, reshedule or cancel their appointment.
 

You may manage your child's appointments after adding them as a dependent.

To add a dependent, please refer to the following steps:

1. On the home screen, tap on "Dependent/Caregiver".
2. Under the "My Dependents" section, tap on "My Children" or "My Family & Friends".
3. Input the requested details and tap on "Save".
4. Upon completion, you will see a "Successfully Added" screen.

To book an appointment for a dependent, please refer to the following steps:

1. On the Home screen, tap on "Dependent/Caregiver".
2. Select your dependent.
3. Tap on "Appointments".
4. Tap on "Add new Appointment".
5. Select the NHG institution and book the appointments.

As your dependent's caregiver, you will be able to view their appointments and bills on NHG Health App.

Dependents above 21 will have to appoint you as their caregiver first. After which, you will be able to access their appointments and perform appointment registration and payment on their behalf. Caregiver would not be able to access the dependent's MC, laboratory results, or other documents.

[For dependents] To manage your caregiver's access, please refer to the following steps:

1. On the home screen, tap on "Dependent/Caregiver".

2. Tap on "My Caregivers".

3. Select your caregiver that you wish to edit access to.

4. Select the respective health record types you wish to give access to your caregiver.

​Yes, however, please note that Healthier SG benefits are only available at your enrolled clinic. 
 

If you choose to enrol in a clinic that is different from where you currently get treated for your chronic condition, and you have pre-existing appointments, please re-schedule your appointments to your newly-enrolled Healthier SG clinic to enjoy the benefits of Healthier SG.​

The process varies depending on your situation:

• ​​NHG Polyclinics' Chronic Patients:

  a. ​​​With appointment during the day of enrolment:

​​Pre-Health Plan review will be done by Care Coordinators, followed by the Health Plan discussion with your doctor during your consultation.

         b. With an upcoming appointment:

The Health Plan will be discussed during your follow-up chronic appointment with us.
 

• NHG Polyclinics' patients with no same-day or upcoming doctor's appointment:

A dedicated Health Plan consultation will be scheduled for you, ensuring sufficient time is provided for your Health Plan discussion.

• New NHG Polyclinics' Patients:

You will have to make an appointment for a Health Plan consultation, via the NHG Health app, available on the Google Play Store or the Apple App Store. 

Additionally, you will be prompted to fill up a form on the HealthHub App, upon successful enrolment with NHG Polyclinics.​

The mammogram servi​​ce is available at all NHG Polyclinics.​

• Monday to Friday (am): 8:00am to 1:00pm

• Monday to Friday (pm): 2:00pm to 4:30pm

• Saturday: 8:00am to 12:30pm

• Sunday & Public Holiday: Closed
   

Registration

• Opens 30 mins before your scheduled appointment time.

• Closes at 4pm (Monday to Friday) and 12pm (Saturday).

• You may register via the NHG Health App (for.sg/get-nhg-health-app​), HealthHub website (healthhub.sg​) or mobile app, or at the Self-Service Kiosks at the polyclinics.

Please be physically present in the dental clinic at your scheduled appointment time. You may be asked to reschedule if you are late.​

Our dental services are strictly on an appointment basis. You may submit an online request to make an appointment for dental services through the NHG Health App (for.sg/get-nhg-health-app​), HealthHub website (healthhub.sg​) or mobile app.
 

Please note that dental appointment slots are released on the 1st day of each month and availability is subject to demand.

If you arrive more than 15 minutes late for your appointment, please approach our counter staff to reschedule your appointment.​

The transfer of dental appointment slots is not allowed. You are encouraged to reschedule your appointment at least three working days before the appointment date in order to safeguard your dental appointment deposit fee.​​

Yes, we provide dental care for patients of all ages.​​​

In addition, we require parents/legal guardians to accompany their child/ward if they require dental treatment not included in the above consent form (e.g. extractions).

DAD is a one-time $8 appointment booking deposit for Dental Service. It is not chargeable to company account or MCPS (third-party payer) but is waived for patients on the Public Assistance or Medicare Assistance Scheme.​
