You can get your child immunised at any of our polyclinics. Our trained nurses also offer developmental screening services for your child.
Important points to note when bringing your child for any immunisation:
Bring along your child's birth certificate on your first visit to the polyclinic and the health booklet at every visit.
It is important to complete the full course of immunisations to ensure that your child has full immunity against the listed diseases.
If you are unable to bring your child for a scheduled appointment, remember to call us at 6355 3000 to schedule for a new appointment. You may use NHG Health App to reschedule appointment.
If your child is going overseas and will be having his or her immunisations administered in another country, you should bring along your child's health booklet to ensure correct continuity of care, administration and documentation of vaccinations received.
Where possible, you should have your child immunised at the same clinic.
A vaccination certificate will be issued after your child has completed his or her schedule of childhood immunisations. The Certificate of Vaccination is one of the documents required for Primary One school registration. Ministry of Health also accepts documentary evidence of immunisation downloaded from the National Immunisation Registry's (NIR) website. Please click here to go to the NIR website.
Singaporeans are able to use their Child Development Account (CDA) account to pay for enhanced vaccinations. If you are keen, please bring along the following documents at your baby's first vaccination visit (at 3 months):
CDA* - NETS card with Pin Number (OCBC/UOB/POSB/DBS)
Latest CDA monthly statement or online CDA statement of account
Parent's/ Trustee's NRIC
Child's original Birth Certificate
*Please ensure you have sufficient funds in your CDA account, otherwise you will have to make a non-refundable upfront payment.
If your child is born overseas:
It is recommended that you call us at 6355 3000 to schedule for an appointment.
Please bring along your child's overseas immunisation records on your first visit, as the attending nurse needs to check and verify your child's vaccination records.

Care after immunisation
Do not rub on the injection site. You may apply gentle pressure with the swab until bleeding stops.
Remain in the waiting area for 15 minutes to observe for adverse reactions such as persistent crying, fits, redness, swelling and pain over the injection site.
Fever and/or rashes are possible reactions to some vaccinations. The nurse will provide the approved fever medication and the Post Vaccination Advice on managing the common reaction to vaccinations. Should your child's fever persist after 48 hours or if there is persistent crying, fits or inflammation at injection site, bring your child to a doctor or to the polyclinic for follow-up.
We monitor the growth of children from birth to four years of age, at regular intervals:

Fever and/or rashes are possible reactions to some vaccinations. The nurse will provide the approved fever medication and post vaccination advice on managing the common reactions to vaccination.Should your child's fever persist after 48 hours or if there is persistent crying, fits or inflammation at the injection site, bring your child to a doctor or to the polyclinic for a follow-up.
If my child has taken some vaccination from my hometown or another clinic, can I continue with the rest in polyclinic? How do I go about doing it?
If you are planning to bring your child to the polyclinic for vaccination, you will have to first update your child's records in the NIR first and then arrange an appointment to bring the child for injections, after the records have been updated.
There are several ways that you can update your child's immunisation record(s)
- NIR website - www.nir.hpb.gov.sg
- Fax - (65) 64381015
- Email - hpb_nir@hpb.gov.sg
To access the website, you will need your SingPass and Password. For more information on obtaining a SingPass, please use the online application service or visit the counters at the CPF Board Office.
For fax or email, you will need the following documents:
- Child's Birth Certificate
- Child's Singapore Citizenship certificate/Dependent Pass/Long Term Visit Pass etc. (if child is not born in Singapore)
- Child's Passport (if Singapore ID document is not obtained)
- Child's Immunisation Records (inEnglish)
- Both parents' NRIC, FIN or Passport
- Both parents' contact phone number and email address
- Residential & mailing address (if different from residential)
You may then call our Contact Centre at 6355 3000 to book an immunisation appointment for your child. Bring along your child’s immunisation records from your hometown or the other clinic. Our nurse will check through the records and if necessary, your child may be referred to consult a doctor so that further assessment can be done. In such cases, consultation fees apply. Please note it is not guaranteed that your child can be given the immunisations on the same day. Our doctors and nurses will advise you accordingly.
Please note that some vaccines your child received in your hometown or previous clinic may not be interchangeable with the vaccines available in our polyclinics. Hence, it is always best to keep to the same clinic as far as possible.
I would like to request for an immunisation certificate. How do I go about doing it?
An immunisation certificate is issued by our nurses when your child completes the childhood immunisation schedule, usually at 18 months of age.
For primary school registration, if you have misplaced your child's immunisation certificate, you may also download your child's immunisation records using your SingPass login ID from the National Immunisation Registry (NIR) at http://www.nir.hpb.gov.sg/nir/eservices/eservice.jsp.
I have misplaced my immunisation certificate and would like a replacement. Is this possible?
We are able to provide a duplicate certificate only for the vaccinations taken in our polyclinics. Please call our Contact Centre at 6355 3000 or email us the following details at enquiries@nhgp.com.sg.
- Birth Certificate number
- Residential Address
