The NHG Population Health Collective (POP Collect) brings together over 60 health and social care organisations in an active co-learning and collaborative network, enhancing the well-being of 1.5 million residents in Central and North Singapore. Through collaborative partnership, we have built an integrated network that truly unites health and social care. Together, we form Communities of Care (CoCs) for integrated place-based care for our residents and enable a social movement of caring communities of residents for health and wellbeing. Together, we add 15 million years of healthy life for our residents.

The collective is guided by the POP Collect Leadership Council, consisting of 15 community partner organisations, social service and national agencies. The council facilitates collective leadership, strategic planning and multi-lateral collaborations to manage the population’s health.
