Population Health Collective
The NHG Population Health Collective (POP Collect) brings together over 60 health and social care organisations in an active co-learning and collaborative network, enhancing the well-being of 1.5 million residents in Central and North Singapore. Through collaborative partnership, we have built an integrated network that truly unites health and social care. Together, we form Communities of Care (CoCs) for integrated place-based care for our residents and enable a social movement of caring communities of residents for health and wellbeing. Together, we add 15 million years of healthy life for our residents.
The collective is guided by the POP Collect Leadership Council, consisting of 15 community partner organisations, social service and national agencies. The council facilitates collective leadership, strategic planning and multi-lateral collaborations to manage the population’s health.

POP Collect gathers annually to strengthen collaborative efforts and encourage the exchange of ideas that drive our collective impact.
A highlight is POP Marketplace featuring showcases and pitch sessions for new ideas and innovative solutions. At POP Marketplace the collective can copy, consult and collaborate with each other for a stronger impact for the community.

15M Awards
Part of the 15M Social Movement to add 15 million years of healthy life, the 15M Awards recognise residents and organisations with significant contribution to the health of their communities in Central and North Singapore.
As a community partner, you can:
- Nominate deserving individuals or groups who go beyond their own households to support, connect, and organise their communities for health and well-being.
- Nominate your organisation to recognise how your work brings together innovation, collaboration, and impact in advancing community health and well-being.

POPPIN’

Health Kampung
Health Kampung is a marketplace of health and social programmes offered by partner organisations in POP Collect, where residents can find and sign up for activities and support for their health and well-being. Health Kampung is accessible via the NHG Health App. Download the app at [click here].

Partners Portal
Partners Portal provides data to partners for better care of residents, and supports in publicity efforts, sign up and tracking of their community programmes for residents. Contact us to discover how your organisation might use partners portal too.
Master Services Agreements
Collaboration with partners is enabled through Master Services Agreements (MSA) that provides an overarching framework for collaborations, project-specific work statements and simplified data sharing. Further collaborations can simply ride on this MSA with additional statements of work. Partners with MSAs are also supported for collaborations by Relationship Managers from NHG Health.
