Start a conversation. Help someone take the next step.

You're seen this before.

A neighbour who has not been looking well.

A friend who keeps putting off that health check or needs a little nudge to stay active.

Sometimes, all it takes is someone who cares enough to point them in the right direction.

The Health Connectors Programme is part of the 15M Social Movement, which aims to add 15 million years of healthy life by strengthening how communities support one another. By encouraging simple, everyday actions, the programme helps make health and well-being support feel more approachable, accessible, and community-driven.

That someone can be you.

A Health Connector is someone who notices when others may need support and encourages small next steps towards better health and well-being. No medical knowledge needed.

What will you gain?

  • Learn simple ways to support and encourage others in their health and well-being
  • Build confidence to start conversations and encourage healthier actions
  • Recognise when someone may need support, and guide them towards available resources

You'll also receive practical guidance on what to say, what to do, and what to avoid.

Who can join?

The programme is open to residents, including those already active in community or volunteer work, who want to support others in simple, meaningful ways.
