At NHG Health, we are committed to building a Community of Care (CoC) in every neighbourhood across Central and North Singapore. Each CoC brings together a network of service partners and activated residents, working collaboratively to deliver needs-based and place-based care. By facilitating  the delivery of care, and health and social programmes, each CoC aims to keep residents are well supported and provide care that matters most to them.

Understanding our residents

As care providers, understanding our residents’ needs  is fundamental to delivering effective health and social care services. In addition, by understanding our residents’ social determinants of health and healthcare utilisation patterns, we can develop more targeted and meaningful interventions that truly address their needs.
