Community of Care (CoC) is a neighbourhood-based support for our residents, bringing together a network of service partners and activated residents, working collaboratively to deliver needs-based and place-based care.

CoCs facilitate the delivery of care, and health and social programmes that are localised to our residents’ needs.

NHG Health is committed to building CoCs with community partners and our network of Community Health Teams and Community Health Posts, to enable our residents age-in-place while receiving the care and support that they need.
