Frailty

Recognising the challenges of our rapidly ageing population, we are taking decisive action to address frailty - a dynamic state of health involving the gradual loss of physiological reserves across physical, cognitive, psychological, and social domains. 

 

Frailty increases vulnerability to adverse health outcomes, but can be prevented, reversed, or delayed through early detection and targeted interventions to optimise functional ability, activity participation, and quality of life.

End-of-Life Planning

Advance Care Planning (ACP) is the process of planning for your healthcare and personal care needs. Should you fall too ill to communicate or make treatment decisions, your ACP helps your care team and chosen spokesperson to make decisions that are in your best interests.

 

We offer ACP Facilitation as a community service, where our community care nurses or trained facilitators guide you through meaningful conversations about your values, care preferences, and treatment choices, ensuring your wishes are respected in times of serious illness or medical crisis.
