Our Be Fit, not Frail (BFF) frailty screening programme is conducted in collaboration with community partners such as Active Ageing Centres (AACs) in Central and North Singapore to help identify and delay frailty progression among seniors.
What the Programme Offers
Through BFF, participants will benefit from:
- A comprehensive screening covering frailty, malnutrition, cognition, falls risks and mood
- A personalised care plan tailored to your health needs
- Guidance on maintaining overall health and preventing frailty
- Referrals to relevant care services, where necessary
Who Should Attend
This programmme is recommended for residents aged 60 years and above who wish to take charge of their health and well-being.

- Request an appointment at our Community Health Post (CHP) here. We will get back to you within a week.
- Call our hotline at 6355 3000 (Mondays – Fridays, 8.00am to 4.30pm; Saturdays, 8.00am to 12.30pm). The hotline is closed on Sundays and Public Holidays.
