
Advance Care Planning (ACP) is the process of planning for your healthcare and personal care needs. Should you fall too ill to communicate or make treatment decisions, your ACP helps your medical team and chosen spokesperson to make decisions that are in your best interests.
We offer ACP Facilitation as a community service, where our community care nurses or trained facilitators will guide you through meaningful conversations about your values, care preferences, and treatment choices, ensuring your wishes are respected in times of serious illness or medical crisis.
