Our care managers are registered nurses specially trained in chronic disease management in primary care setting. The care manager works closely with your doctor, advanced practice nurse, allied health members and yourself to manage your chronic conditions. The care manager provides the following care to you:

• Obtain a thorough health history and assess your level of disease control
• Identify factors that contribute to your sub-optimal disease control
• Provide education on lifestyle modification and teach self-management to empower you to manage your health
• Monitor your condition and work with your doctor to review your plan of care regularly.
