  1. Fees stated for dental services are for consultation only and does not include procedural cost. During the consultation, the dentist will assess your oral conditions, discuss the treatment plan, advise on the procedural cost, and schedule a next appointment if needed.
  2. Dental emergencies refer to cases without an appointment and/or suffering trauma, severe pain, swelling and bleeding only.
  3. Bill generated at Polyclinic is accurate at the time of print, and may be revised if there are any charges not captured at the time of print.
  4. For all other fees and charges, please call 6355 3000, use our Enquiry Form​, or approach our counter staff at the polyclinic for assistance before proceeding with the service.​

  • Enter UEN number “200002150HNP2” 
  • Enter the amount and the bill number in the “Bill Reference No.” field.
