Collection Instructions:
1. Complete HSA form (Annex A-3 Panel 1 of HSA form).
2. Draw sufficient blood into green top tube.
3. Gently invert the tube 8 - 10 times.
4. Send form and specimen to the laboratory as immediately.
Containers must be sealed for medico-legal cases, in order for the evidence to be legally defensible in court.
