Our flagship caregiver support programme, CarerLink, is a collaboration between WH and Care Corner to provide post-discharge training, support and respite for caregivers requiring follow through after their care recipients returns home.

​By working with Nursing and Welfare Homes in the north, we can tighten communications of residents’ admission, and co-develop post-discharge follow-up workflows. We started this initiative with Orange Valley Nursing Home, MWS Christalite Methodist Home and Metta Home, and hope to extend this to the rest of our partners.

We aim to create a robust care ecosystem that reflects our commitment to collaborative health-socio care through consolidating details on clinical services, community initiatives, and support resources. This will equip our clinicians to design the best possible care for our patients beyond the hospital. ​

TriGen is a tri-generational community care programme to support WH high-risk patients through home-based care to address their health and social needs. In partnership with NUS Medical students, the programme empowers student volunteers to provide meaningful home care support through regular home visits and meaningful engagements and activities.

​Through caring for our discharged patients, this creates valuable learning experiences for student leaders and students. This programme is supported by North West CDC.​ ​
