Primary Care
Overview
We collaborate with General Practitioners and Polyclinics to facilitate patient access to care for acute ailments and the management of chronic conditions. Our aim is that each patient is assigned a dedicated Primary Care physician, to ensure seamless patient care integration during transition between community and hospital settings.
By ensuring every patient has a designated Primary Care physician, we foster continuity of care and enhance patient outcomes across different healthcare environments.
As part of the national Healthier SG plan (Healthier SG), we collaborate with GPs to assist residents in achieving their health goals and connect them with community resources such as programmes, activities and services.

Khoo Teck Puat Hospital and NHG Population Health Primary Care Office work closely with primary care partners to co-create and implement various enablers and programmes that provide transitional care and support for patients and residents in the community.

Community Resources for Patients & Residents
We work with community care partners to offer a suite of community programmes and services to empower residents to better care for themselves and their loved ones. These include workshops and activities, community screenings, community health post services and more.
