Frequently Asked Questions |
What is a medical report?
A medical report is a structured document, usually in a standard form provided by an insurance company or relevant authority. It contains medical information that must be completed by an assigned doctor.
What documents can I submit for my outpatient visit or hospitalisation claim?
For most insurance claims, you may submit:
Why is there a charge for a medical report?
Preparation of a medical report requires the doctor to review the patient’s complete medical records and provide clinical opinions as requested. This work is performed separately from the patient’s outpatient consultation or hospital stay. As such, consultation fees and hospitalisation bills do not include the cost of medical reports. The fee charged covers the hospital’s administrative processing and the doctor’s professional time and input.
Can MediSave or MediFund be used to pay for a medical report?
No. Payments for medical reports are not claimable under MediSave or MediFund.
I was seen by two different doctors at Khoo Teck Puat Hospital. Can I request one consolidated medical report?
If you were reviewed by different doctors for separate medical conditions, each doctor is required to prepare a medical report specific to the care and treatment provided. Therefore, separate medical reports must be requested for each specialty.
I have visited the hospital for various reasons. Which medical report should I be requesting?
A fee applies to each medical report requested. You are advised to request only the report that is relevant to the purpose of your insurance or administrative claim. |
