KTPH Advanced Internal Medicine Specialists provide comprehensive, person-centred care for patients with complex medical needs—across the inpatient stay and beyond discharge in our outpatient clinics.

We care for patients with multiple comorbidities and or diagnostic uncertainty, bringing both breadth and depth of internal medicine expertise to untangle diagnostic dilemmas and optimise long-term management. We take a holistic approach that considers not only medical issues, but also functional status and psychosocial, emotional, and social factors that shape recovery and wellbeing.

In the hospital, we support our clinical teams through blue-letter inpatient consultations, offering timely expert input for complex cases, multisystem problems, and challenging treatment decisions.

We also run the Medicine Short Stay Ward, providing focused care for patients with acute conditions who may be suitable for safe early discharge—typically up to three days— and timely transition back to the community with resources available for early review outpatient.

After discharge, we provide continuity of care for complex patients, coordinating follow-up, medication optimisation, and monitoring to reduce complications and prevent avoidable readmissions. We accept new referrals from polyclinics, general practitioners, and the Emergency Department for patients who require deeper assessment, diagnostic clarification, and integrated internal medicine care.

We partner closely with subspecialty teams and allied health professionals to deliver seamless, coordinated care, including step-down and transitional models such as Hospital-at-Home (in collaboration with KTPH@Home) and ongoing recovery support through community hospital rehabilitation (in collaboration with Yishun Community Hospital).
