
The Department of Geriatric Medicine (GRM) provides medical services for conditions related to ageing for older persons in the inpatient, specialist outpatient clinics and community care settings.
We assess and address the needs for older persons with multiple medical problems related to ageing as well as geriatric syndromes such as cognitive impairment, falls and balance disorders, osteoporosis and fragility fractures, incontinence, immobility, frailty, advanced progressive and terminal illnesses, and rehabilitation medicine in both inpatient and outpatient specialist clinic.
In community, we work closely with community partners such as home care programmes, day care facilities, community hospitals, nursing homes and hospices to continue seamless care.
Our vision is to add healthy life to older persons through comprehensive clinical care, while seeking continual improvement through research and education.
Our Services
Inpatient
- Older persons are admitted to Acute Care of the Elderly (ACE) wards consist of multi-disciplinary team of specialist doctors, nurses, physiotherapists, occupational therapists, speech therapists, dietitians, pharmacists, psychologists and medical social workers are available to provide integrated assessment and management.
- The Care of the Acutely Ill Mentally Infirmed Elderly (CAMIE) ward is a specialised ward for patients with hyperactive delirium or challenging behaviour complicating their acute medical problems. CAMIE adopts a person-centred care philosophy that upholds the dignity and autonomy of the patient and promotes quality of life through enhanced care that adopts a no physical restraints policy.
- The 16-bedded Geriatric Short Stay Ward (GSSW) houses patients who only require short inpatient stay and accepts admission from Early Cognitive Impairment Clinic to receive the latest dementia treatment.
- Palliative Care ward provides palliative care input to patients with advanced progressive life-limiting illnesses requiring assistance with end-of-life care.
- Ortho Geriatrics and Geriatric Perioperative Medicine Service collaborate with surgical team to optimise and manage older persons who admitted for hip fractures and those who require emergency and elective surgery.
- Our Rehabilitation medicine team continues to provide consultation and rehabilitation service in Yishun Community Hospital (YCH).
- 6th vital sign cognition team led by our geriatricians and advanced practice nurses actively engage patients with behaviour of concern in other wards using Humanitude and person-centered care approach to ensure they are well taken care of during their hospitalisation.
Outpatient Clinics
TeleGeriatrics
TeleGeriatrics provide specialist outpatient care via video consultation, allowing assessment to be done in the patient's own living environment without the need to travel to hospital.
This service is available for older patients who are already on our follow up if there is a family caregiver who is able to support and patient does not require investigations or treatment that can only be delivered at the hospital. Blood tests can be arranged at home and medications will be delivered after the consultation.
For patients who reside in nursing homes, integrated care is achieved by providing assessment and diagnosis of patients in the nursing home with their care staff. Trained nurses in the nursing home act as the hands of the doctors by performing simple clinical examination on patients and reporting their findings to the doctor in KTPH. Doctors are then able to advise on clinical management without being physically present at the nursing homes.
Education and Fellowship Programme

For Old Time's Sake is an initiative by KTPH Geriatric Medicine dedicated to empowering older adults to take an active role in their physical, mental, and emotional wellbeing as they age into their golden years. Through interactive talks and practical resources, we aim to inspire participants to live healthily, meaningfully, and with confidence. Our recent sessions included:
Successful Ageing – Practical nutrition and exercise strategies to maintain health, as well as guidance on health screening.
- Healthy Mind, Happy Life – Insights on brain health and ways to reduce dementia risk, alongside tips for maintaining mental wellness.
