Geriatric Emergency Medicine Comprehensive Assessment & Responsive Engagement

 

Our Geriatric Emergency Medicine (GEM) team works closely with our Emergency Department to assess and manage medical emergencies for elderly patients. This multi-disciplinary approach ensures that these patients receive services that are best suited for their care and minimises admissions and re-visits to the hospital.

GEM Nurse
GEM nurses are specially trained to care for older adults. They work closely with patients and their families throughout the stay. They conduct a comprehensive geriatric assessment to evaluate patients' medical issues, memory, mobility, emotional health, home situation, daily activities, fall risks and any signs of confusion. Working with A&E and Geriatric doctors, GEM nurses develop care plans for discharge and link patients to community services to ensure they receive the right support.

Q: Who attends to geriatric emergency patients?

A: Our team comprising medical professionals from the Emergency Department, Geriatric Medicine and Rehabilitation Service will care for the patients.

 

Q: How long is a patient expected to stay?

A: Patients can expect to stay at A&E for a day. Thereafter, they will be either discharged directly, discharged with referral to community care services or specialist outpatient clinics, or admitted to the ward for further treatment if necessary.

 

Q: Are visits allowed?

A: Yes, up to two visitors are allowed at any one time during these times: Noon-2pm and 5pm-8.30pm.
