WH@Home provides care for suitable patients to recover in the comfort of their home instead of the hospital.

Our WH@Home care team will closely monitor your condition through daily teleconsultations. We will also do home visits when necessary to provide medical and nursing care as well as rehabilitation therapy to help in your recovery.

Eligibility



Please consult your doctor to determine your eligibility for WH@Home.

Generally, you should:
  • Be above 16 years old
  • Be medically stable but still require hospital-level care
  • Be independent or have adequate caregiving support at home
  • Have internet access at home to facilitate teleconsultations

Conditions suitable for WH@Home:
  1. Cellulitis (bacterial infection of the skin)
  2. Urinary Tract Infections
  3. Vomiting and Diarrhea
  4. Chest infections e.g. pneumonia
  5. Dengue
  6. Poorly controlled blood sugar levels that require hospitalisation
  7. Poorly controlled blood pressure levels that require hospitalisation
  8. Rhabdomyolysis (rapid breakdown of damaged muscle cells)
  9. Fluid Overload from heart and kidney disease
  10. Gout/ Inflammatory Joint Disease

Here’s what you can expect:


  1. Programme briefing by the WH@Home care team

  2. Head home with loaned devices, medications, and medical supplies

  3. Daily check-ins via teleconsultations or home visits
    (A nurse will visit you either on your enrolment day or the next day.)

  4. Daily submission of vital signs

  5. Once discharged, receive discharge documents and return loaned devices
    (Arranged by the WH@Home care team.
    )

  6. Collect discharge medications at WH Pharmacy or opt for home delivery
    (WH Pharmacy is located at Tower B, B1.)

Charges



Similar to a hospital stay, the cost of WH@Home depends on your condition and recovery time.

WH@Home is claimable under MediSave and MediShield Life as well as most private healthcare insurers (subject to your healthcare insurer’s terms and conditions).

Frequently Asked Questions


  1. My doctor spoke to me about receiving care at home under WH@Home, and I have agreed. What next?

    Upon enrolment, our WH@Home care team will brief you on what to expect over the next few days. When ready, you can return home with the necessary medications and loaned equipment.

    Our nurse will make a first home visit either on the night of enrolment or the following morning. Subsequent daily check-ins are via teleconsultations or home visits if necessary.

    The WH@Home care team will advise when you are ready for discharge and will arrange for the delivery of your post-discharge documents, as well as the return of loaned devices. You may collect your medication at WH Pharmacy (Tower B, B1) or opt for home delivery.

  2. What if my condition becomes worse?

    Our dedicated multi-disciplinary team is available 24/7 to provide immediate support. You may call the WH@Home hotline so we can assess your condition and take the necessary steps to ensure you receive the appropriate care.

  3. Is WH@Home more expensive than an inpatient hospital stay?

    Charges for WH@Home are similar to or lower than a normal hospital ward.

  4. How long do I need to stay in WH@Home?

    The length of stay depends on your condition and recovery. Our WH@Home care team will advise and provide guidance along the way.

  5. What if I have other WH appointments while I am still in WH@Home?

    For essential appointments related to your care in WH@Home (e.g. clinic appointments, radiology scans), transport services will be arranged for you and our team will meet you on your arrival at WH.
