Biodata

Adj Asst Prof Tan is a Family Physician, Senior Consultant in the Department of Continuing and Community Care in Tan Tock Seng Hospital. He is currently the Head of Department.

His clinical works include outpatient clinic, home-based medical care, and nursing home consultancy.

Adj Asst Prof Tan's interests are in the areas of sleep medicine, post-discharge care, and working with community partners in the holistic management of the patients in the community. As the clinical lead for the Community Health Team Service – NHG Population Health, he assists in the planning and implementation works to achieve the physical, mental, and social well-being of the patients enrolled in the service.

He is also involved in the clinical teaching of medical undergraduates, nurses, and doctors doing their post-graduate trainings in Family Medicine and Graduate Diploma in Geriatric Medicine.
