Biodata

Dr Tai Zu Huang is a Family Physician, Consultant with the Community & Continuing Care Department at Tan Tock Seng Hospital, Integrated Care Hub. He provides clinical leadership in post-acute and rehabilitation medicine, with a core focus on optimising recovery and supporting ageing-in-place for older adults transitioning from acute hospitals back to the community.

Dr Tai’s practice spans subacute rehabilitation, post-surgical orthogeriatric care, stroke recovery, and complex chronic care management. He works closely within a multidisciplinary team to deliver holistic, person-centred rehabilitation, aiming to enhance functional outcomes, reduce care fragmentation, and improve patients’ quality of life. His key areas of interest include post-acute and continuing care, integrated care models, frailty and functional recovery, and transitional care services that reduce hospital readmissions and support patients and caregivers through recovery.

Beyond clinical work, Dr Tai is actively involved in medical education. He teaches at both undergraduate and postgraduate levels, including the Lee Kong Chian School of Medicine, Yong Loo Lin School of Medicine, NHG Family Medicine Residency Programme, and the College of Family Physicians Fellowship Programme.
