Dr Soh Ling Ling is a Family Physician, Consultant with the Department of Community & Continuing Care at Tan Tock Seng Hospital. She works with the Community Health team (a multi-disciplinary service) of NHG Population Health, supporting care of patients transitioning from acute hospitals back to the community. She contributes her time as a senior doctor at TTSH Integrated Care Hub (TTSH ICH) which provides post-acute inpatient care and rehabilitation to patients. She also participates in the teaching of Family Medicine residents and medical students during their attachments to the department.

Dr Soh has experience in differing settings of primary care, having practised in the polyclinics, as a locum doctor in general practitioner clinics, as a volunteer doctor in a non-profit clinic in East Asia, as well as in community hospitals in Singapore.

Dr Soh believes in empowering and supporting caregivers to provide holistic care to patients.
