Dr Lim Hong Hwee is a Family Physician, Associate Consultant at NHG Yishun Polyclinic. She graduated from the Yong Loo Lin School of Medicine, National University of Singapore in 2016 and completed her Family Medicine training in 2021 with a Master of Medicine (Family Medicine).

Dr Lim has a strong clinical interest in geriatric care and chronic disease management. She is committed to providing comprehensive and compassionate care tailored to the needs of older adults and patients with long-term conditions.

She is actively involved in education as a Core Clinical Faculty Member with the NHG Family Medicine Residency Program. She also serves as Co-Chairman of the NHG Mock Master of Medicine Examination Organising Committee for AY2024–2025.

Within her clinic, Dr Lim leads a clinical teamlet and is dedicated in providing quality care and team-based practice. She is proficient in English and Mandarin.
