Dr Lye Yik Fan is a Family Physician at NHG Serangoon Polyclinic. Dr Lye began his medical journey at the University of Malaya, Malaysia, where he earned his MBBS in 2009. Motivated by a deep interest in children’s health, he pursued postgraduate training in paediatrics, eventually earning his Membership of the Royal College of Paediatrics and Child Health (MRCPCH) in the United Kingdom in 2015. He continued to practice in Paediatrics until 2016, developing expertise in paediatric care.

In 2016, Dr Lye made a pivotal career shift to Family Medicine, driven by a desire to extend his care beyond children and serve patients of all age groups. To further enhance his expertise in primary care, Dr Lye completed the Graduate Diploma in Family Medicine in 2021 and obtained a Master of Family Medicine from the National University of Singapore in 2023. He is a certified Family Physician, accredited in 2021. With a strong commitment to holistic care, he has developed a keen interest in chronic disease management, children’s health, and preventive medicine.

As a dedicated clinician and educator, Dr Lye is an active member of the Paediatric Special Advisory Group in NHG Polyclinics, where he contributes to emergency care planning and policy development for paediatric populations.

Beyond clinical practice, Dr Lye is an adjunct clinical tutor at both the Lee Kong Chian School of Medicine and the Yong Loo Lin School of Medicine, National University of Singapore. He is actively involved in clinical teaching for medical students in primary care settings, helping to shape the next generation of physicians. In addition, he is a clinical tutor with the College of Family Physicians Singapore, where he teaches and mentors Master of Medicine (Family Medicine) candidates.

In the area of leadership, Dr Lye currently serves as the Teamlet Lead at NHG Polyclinics. In this role, he leads multidisciplinary team-based care initiatives aimed at improving continuity of care and health outcomes for patients with complex chronic conditions.

Dr Lye’s career reflects a strong dedication to person-centred healthcare and medical education. His combined clinical, academic, and leadership contributions underscore his commitment to improving the quality and accessibility of primary care in Singapore.
