Dr Moses Tan Mong Heng is an Family Physician, Associate Consultant at NHG Geylang Polyclinic. He graduated from National University of Singapore with an MBBS degree in 2014. He subsequently completed postgraduate training in Family Medicine and obtained the Master of Medicine (MMED) in Family Medicine (Singapore) in 2019, Membership of the College of Family Physicians Singapore in 2021 and Fellowship of the College of Family Physicians Singapore in 2023.
Dr Tan has developed a keen interest in diabetes management and multidisciplinary care. He has been heavily involved in making significant contributions to the field. He currently co-leads the NHG Polyclinics Endocrine Specialty Advisory Group and contributes to NHG Diabetes Education Workgroup. He is passionate about educating both healthcare professionals and patients on diabetes through collaborative and multidisciplinary care.
Beyond clinical practice, Dr Tan is a passionate educator and mentor. He serves as a Clinical Tutor at Lee Kong Chian School of Medicine and Yong Loo Lin School of Medicine. He is also an examiner for MMED (Family Medicine) and Lee Kong Chian School of Medicine Year 4 Clinical Exams. He is also involved in mentoring junior colleagues, including NHG Polyclinics Junior Family Physicians and medical officers.
In the area of leadership, Dr Tan serves as the Deputy Head at NHG Geylang Polyclinic, where he has been instrumental in manpower management and key clinic initiatives including the Geylang War on Diabetes and optimizing health education for patients with newly diagnosed diabetes. He is also the co-Lead for Group Education and eMpowerment (GEM) at NHG Polyclinics.
He is also active in research, recently publishing a qualitative paper on "Challenges of care coordination for complex patients among family medicine residents in an outpatient ambulatory clinic" in BioMedical Central in May 2024.
Recognizing the importance of continuous professional development, Dr Tan has also completed numerous courses and certifications, including training in Artificial Intelligence in Healthcare, Clinical Practice Improvement Practice and Root Cause Analysis.
Through his multifaceted contributions to clinical practice, education, research, and leadership, Dr Tan continues to make valuable contributions to public healthcare in Singapore.
