Biodata
Dr Nurul Azyan is a Family Physician, Associate Consultant at NHG Kallang Polyclinic. She graduated from the NUS Yong Loo Lin School of Medicine in 2014 and completed her postgraduate training with a Master of Medicine (Family Medicine) from NUS in 2018. She also holds a Graduate Diploma in Mental Health (2023) and is a member of the College of Family Physicians Singapore.
Committed to delivering patient-centred care, Dr Azyan leads a Teamlet at her polyclinic, adopting a relationship-based, team approach to managing patients with chronic diseases. She also heads the Health and Mind Service in her clinic, supporting patients with mental health needs, and anchors the RELATE team — a multidisciplinary team dedicated to managing and coordinating care for complex patients in the community.
In the area of leadership, Dr Azyan has taken on key administrative roles. She serves as the Key Trainer at her clinic (2022–2025), overseeing the learning and professional development of colleagues. In addition, she is the Deputy lead in the Specialty Advisory Group (SAG) for Psychiatry within NHG Polyclinics, where she actively advocates for strengthening mental health services in primary care.
Beyond clinical practice, Dr Azyan is a passionate educator and mentor. She is a Clinical Lecturer with the NUS Yong Loo Lin School of Medicine and an Adjunct Teaching Faculty, Clinical Teacher, and Clinical Practice Facilitator with the Lee Kong Chian School of Medicine. She is also a Physician Faculty member with the NHG Family Medicine Residency programme and a tutor for the Graduate Diploma in Family Medicine under the College of Family Physicians Singapore.
In recognition of her contributions, Dr Azyan has received several awards, including the Houseman PGY1 (2014–2015) Highly Commended Award (TTSH) and the NHG Teaching Award for Senior Doctors (2024). She also received the Champion Award for the NHG Polyclinics OURCARE Quality Improvement Project in 2018, where she was part of the team who successfully improved diabetic retinal photography completion rates among patients in a polyclinic. These recognitions reflect her dedication to advancing medical excellence, education, and patient outcomes.
