Dr Seitoh Yu Ting, Elizabeth is an Associate Consultant, Family Physician at NHG Polyclinics. She graduated with an MBBS from the Yong Loo Lin School of Medicine, National University of Singapore in 2013. She subsequently obtained the Graduate Diploma in Family Medicine (GDFM) in 2018 and the Master of Medicine (Family Medicine) in 2020 and is accredited in Family Medicine.

To further her interest in mental health, Dr Seitoh completed the Graduate Diploma in Mental Health (GDMH) in 2022. Her clinical focus is on integrating mental health services within primary care. She is a member of NHG Polyclinics Psychiatry Specialty Advisory Group (SAG) and serves as the Health & Mind Clinic (HMC) Lead at NHG Yishun Polyclinic.

Committed to education and mentorship, Dr Seitoh holds adjunct academic appointments as Clinical Teacher at the Lee Kong Chian School of Medicine and Clinical Tutor at the Yong Loo Lin School of Medicine, National University of Singapore. She is actively involved in postgraduate training as a Tutor for the Graduate Diploma in Family Medicine and provides in-clinic coaching for both GDFM and MMED candidates.

At NHG Yishun Polyclinic, Dr Seitoh holds multiple leadership roles. She is the Clinic Lead for Lighter Life, PACt (Patient Activation through Conversations), and GEM (Group Education and eMpowerment) which are support programs that enhance care for patients with Obesity and Diabetes Mellitus. She also serves as the Lead Doctor for a clinical teamlet and is active in RELATE (RELationship-based health and social inTEgration), a programme supporting patients with complex needs in the community. Her work spans team-based care training, mental health service delivery, and chronic disease management in the primary care setting.

In recognition of her contributions to team-based care and quality improvement, she has received the NHG Recognition Awards – Team Recognition Award (Gold) in 2023 under PACt, and the NHG OurCare Quality Improvement Project Award (Outstanding) for her initiative to optimise anticoagulation in patients with Atrial Fibrillation in Primary Care.
