Dr Lee Wei Zhen Candice is a Family Physician and Associate Consultant at NHG Serangoon Polyclinic. She graduated from Trinity College, University of Dublin with an MB, BCh, BAO degree in 2011 and subsequently completed postgraduate training in Family Medicine, obtaining the Graduate Diploma in Family Medicine (2016), Master of Medicine in Family Medicine (2018), and the Graduate Diploma in Family Medicine Dermatology Programme (2020). She is a Collegiate Member of the College of Family Physicians Singapore (MCFPS), awarded in 2021.
To further enhance her expertise, Dr Lee actively contributes to the Cardiology Specialist Advisory Group at NHG Polyclinics, where she serves as Deputy IC. With a strong commitment to patient care, she has developed a keen interest in improving collaborative care models between specialists and family medicine to achieve better patient outcomes. She also strives to improve quality of care by streamlining processes to enhance clinical effectiveness.
As a distinguished expert, Dr Lee is a Collegiate Member of the College of Family Physicians Singapore. She plays a key role in clinical practice transformation and the development of integrated care pathways.
Beyond clinical practice, Dr Lee is a passionate educator and mentor. She serves as a Clinical Tutor at the Lee Kong Chian School of Medicine and contributes actively to undergraduate and postgraduate medical education, including teaching for the Graduate Diploma in Family Medicine and the Master of Medicine (Family Medicine) programmes. She is also a committee member for the Graduate Diploma in Family Medicine Mock Exam Committee, supporting the training and assessment of future family physicians.
In the area of leadership, Dr Lee has taken on key administrative roles. As Deputy IC of the Cardiology Specialist Advisory Group at NHG Polyclinics, she has been instrumental in strengthening primary-specialist collaboration and enhancing chronic disease management. Currently, she also contributes to quality improvement initiatives that focus on value-based care and patient-centred clinical excellence.
Recognising her outstanding contributions to public healthcare, Dr Lee has received accolades such as the Excellence in Action Award (Team) in 2024 for the NHG Tele-AF collaborative project and the NHG Team Recognition Award (GOLD) in 2023 for the NHG Polyclinics Patient Activation through Conversations (PACt) initiative. These recognitions reflect her dedication to advancing medical excellence and improving patient outcomes.
