Dr Koh Zhong Wei Jeremy is a Family Physician, Consultant at NHG Hougang Polyclinic. Dr Koh graduated from University College Cork, Ireland with MB, BCh, BAO (Honours) in 2011. He subsequently completed postgraduate training in Family Medicine with a Master of Medicine (Family Medicine) from the National University of Singapore in 2017 and was awarded Fellowship of the College of Family Physicians Singapore (FCFP) in 2021. As a recognition of his excellence, he was awarded the Dr Koh Eng Kheng Gold Medal during his FCFP.

Dr Koh serves as a member of the Renal Specialty Advisory Group, contributing his expertise to enhance care protocols and management strategies for patients with kidney-related conditions in the primary care setting. He is actively involved in medical education, participating in both undergraduate and postgraduate training to nurture the next generation of healthcare professionals.
