The Central-North Primary Care Network (CN-PCN) is a cluster-led PCN by NHG Health, comprising over 200 GP clinics. GPs in our network is supported by a dedicated team including a Clinical Lead, Admin Lead, GP Sub Leads, Nurses, Care Coordinators and Relationship Managers.

CN-PCN provides nursing, ancillary and support services to patients with chronic conditions at GP clinics and community nodes. Through this network, we bring care closer to residents in the Central and North region, enhancing accessibility and continuity of care.
