The Department of Continuing and Community Care provides medical oversight for patients in TTSH Integrated Care Hub (TTSH-ICH)'s Intermediate Rehabilitation wards, working within a multi-disciplinary team of nurses, therapists, and allied health professionals.

Through a holistic approach that includes both conventional and innovative therapies, we help patients recovering from acute illnesses or post-surgical procedures to regain their functional independence and physical capabilities needed for daily activities, supporting their successful reintegration into their homes and community.
