NHG Polyclinics is licensed under Healthcare Services Act by Ministry of Health to provide and offer suitable patients the option of having remote consultations with our Care Team of doctors, nurses and allied health professionals, from the comfort of their own homes. Please see below for a list of the Tele-Health Services we currently offer for eligible patients.

Primary Tech-Enhanced Care (PTEC)
Eligible patients with high blood pressure or diabetes may be offered the option of enrolling under the PTEC Home BP Monitoring and/or PTEC Home Diabetes Monitoring Programme. Patients enrolled under these programmes will monitor their blood pressure and blood sugar levels at home using Bluetooth-enabled blood pressure or blood glucose meter.

Tele-Dietetics
Patients with chronic diseases, as well as paediatric patients with feeding and nutritional needs, are offered a video consultation with NHG Polyclinic’s dietitians. Besides convenience and time-savings to patients, Tele-Dietetics allows our dietitians to develop specific dietetic interventions that are practical in a patient’s familiar environment. | ![]() |

Tele-Dot
Tele-DOT or Tele-Directly Observed Therapy allows eligible patients with tuberculosis to substitute their face-to-face visit to the polyclinic with a video consultation with a nurse. This service allows patients to complete their therapy from the safety and convenience of their homes.
Note: As part of the Singapore Tuberculosis Elimination Programme (STEP), NHG Polyclinics has been providing Directly Observed Therapy (DOT) to patients with tuberculosis.

Tele-MSW
Eligible patients requiring psychosocial case management and counselling may be offered video consultations with NHG Polyclinics' medical social workers. This service enables our medical social workers to provide support and intervention to patients and their caregivers from the comfort of their homes, reducing the need for travel to the clinics. | ![]() |

Tele-Physiotherapy
Eligible patients with existing physiotherapy review appointments may be offered therapy via video consultation with an NHG Polyclinics physiotherapist. This provides patients with convenient access to physiotherapy services without having to leave their homes. Patients’ movements and prescribed exercises are properly guided during the video consultation. | ![]() |

Tele-Psychology
Eligible patients with existing psychologist review appointments may be offered the option of a video consultation with NHG Polyclinics’ psychologist. This service enables psychologists to provide crucial mental health support and intervention to patients from the comfort of their homes, reducing the need for patients to travel to the clinics. | ![]() |

Tele-Wound Monitoring
Patients who require frequent care for wounds such as superficial pressure injuries or post-surgical wounds may be enrolled into NHG Polyclinics’ Tele-Wound Monitoring service. This service allows wound nurses to provide remote wound monitoring and wound assessment, enabling patients to self-manage their wounds at home. Follow-ups are conducted over a telephone call. | ![]() |

Video Consultations with Doctors
Eligible patients with well-controlled chronic conditions such as diabetes, hypertension and high cholesterol are offered the option to review their medical conditions and laboratory test results via video consultation with our Family Physicians.
Medications prescribed during the consultation can be delivered to the patients’ homes. This service allows patients to manage their chronic conditions from the comfort of their homes, reducing the need for travel to the clinics.

Video Consultation with Nurses
Patients with lipid disorder post coronary risk screening and medication titration follow-up can be offered the option of video consultation with Care Manager – a nurse specially trained in chronic disease management.
