b) Driving License - 4A, 4 and 5
c) Vocational Driving License (e.g. Omnibus Driver, Taxi Driver, General Bus Driver, Bus Attendant)

Please call our Contact Centre at 6355 3000 to arrange for a doctor consultation and a nursing screening appointment for this service.

For existing NHG Polyclinic patients who are:


• Receiving ongoing chronic care, and/or
• Enrolled in Healthier SG (HSG)

Note: Patients are expected to bring their own assessment/application forms for all medical screening and assessment services.

The following medical screening and assessment services are not provided at NHG Polyclinics:

 

Employment & Occupational Health

  • Pre-employment screening (e.g. medical examination for domestic/foreign workers)

  • Medical examination for high-risk activity sports (e.g. diving, aviation, mountain climbing, outward bound school activities)

  • Food Handler Screening

 

Education & School-Related Assessments

  • Pre-admission screening for schools and/or medical assessment for application to Special Education (SPED) Schools

 

Elderly Care & Disability Support

  • Interim Disability Assistance Programme for The Elderly (IDAPE)

  • ElderShield assessments

  • Caregiver Training Grant assessments

  • Special Needs Saving Scheme assessments

 

Personal & Legal Services

  • Pre-marital Screening

  • The Lasting Power of Attorney

 

Insurance & Financial Planning

  • Medical examination for enrolment to plans/programmes (e.g. insurance purchase)
