1	Driving License Medical Examination a) Driving License - 2B, 2A, 2, 3A and 3 b) Driving License - 4A, 4 and 5 c) Vocational Driving License (e.g. Omnibus Driver, Taxi Driver, General Bus Driver, Bus Attendant)	For existing NHG Polyclinic patients who are: • Receiving ongoing chronic care, and/or • Enrolled in Healthier SG (HSG)
2	Car Park Label Scheme for Persons with Physical Disabilities	Applicants with physical disability or medical condition that limits mobility
3	Application for Government Financial Assistance Schemes: a) SG Enable Financial Assistance Schemes and Disability Services (e.g. disability home, day activity centre, sheltered workshop) b) Public Transport Concession Scheme for Persons with Disabilities c) Claim of Disability-Related Tax Reliefs d) AIC – Pioneer Generation Disability Assistance Scheme (Only for Pioneer Generation)	Applicants with physical disability requiring a functional assessment report
4	Application for Home Care Arrangement a) Voluntary Welfare Organisation (VWO) Nursing Home Placement and Respite Care b) AIC Centre-based services (e.g. Day Rehabilitation, Day Care, Dementia Day Care) c) Home-based services (e.g. Home Medical, Home Nursing, Home Therapy, Home Help, Home Care) d) MSF Destitute Home and VWO Sheltered Home	Applicants requiring assessment for home care or institutional care

Medical Screening and Assessment Services NOT Available at NHG Polyclinics

The following medical screening and assessment services are not provided at NHG Polyclinics:

Pre-employment screening (e.g. medical examination for domestic/foreign workers)
Pre-admission screening for schools and/or medical assessment for application to Special Education (SPED) Schools
Medical examination for high-risk activity sports (e.g. diving, aviation, mountain climbing, outward bound school activities)
Interim Disability Assistance Programme for The Elderly (IDAPE)
Eldershield assessments
Caregiver Training Grant assessments
Special Needs Saving Scheme assessments
Medical examination for enrolment to plans / programmes (e.g. insurance purchase)
Pre-marital Screening
Food Handler Screening
The Lasting Power of Attorney
