Please note that cancellation and rescheduling must be done at least 3 working days before your appointment date.

Important to Note:

Punctuality

  • Please be on time.
  • For patients who have registered on HealthHub, the number shown on HealthHub is indicative of the number of patients ahead of you.
  • As queue number may not be called in sequence, we recommend that you wait physically in the dental clinic during your scheduled appointment time.
  • Patients who are late may be asked to reschedule their appointment. (A refundable deposit fee of $8 is required when you book a dental appointment).

Appointment Registration

  • Please register via the NHG Health App (for.sg/get-nhg-health-app), HealthHub website (healthhub.sg) or mobile app, or at a Self-Service Kiosk before heading to the Dental clinic. To reduce crowding in the clinic, please come only at your appointment time. Registration will only be available 30 minutes before your scheduled appointment time.

Documents to Bring:

  • NRIC / Passport / Employment Pass / Work Permit, and any referral letter, medical memo or benefit card.

Consent for Patients Below 21*

  • A parent or the legal guardian of patients below the age of 21 will need to complete the consent form. Dental treatment will not be rendered if the consent form is not duly completed and signed. The consent form can be downloaded here. In addition, we require parents/legal guardians to accompany their child/ward if they require dental treatment not included in the above consent form (e.g. extractions).
  • *NS men/Foreign Workers (Work Permit and S Pass Holders) do not require a consent form.

Cancellation/Rescheduling of Appointment

  • Please do so via the NHG Health App (for.sg/get-nhg-health-app), HealthHub website (healthhub.sg) or mobile app. Your Dental Appointment Deposit (DAD) (if applicable) will be forfeited if you are late for more than 15 minutes or do not turn up for your appointment or cancel/reschedule less than 3 working days before your appointment.

Dental Appointment Deposit (DAD) Fee

  • The introduction of appointment deposit fee system has helped us serve you more efficiently as patients are encouraged to keep to their appointments or cancel/re-schedule them in advance so that these appointment slots can be given to others who need our dental services. For more information, please refer to our FAQ.
