Dr Yong Yan Zhen is a Family Physician, Associate Consultant at NHG Sembawang Polyclinic. She has clinical interests in caring for patients with dermatological and complex chronic conditions. 

Dr Yong holds a Bachelor of Medicine and Bachelor of Surgery degree from the National University of Singapore (NUS). She completed the Master of Medicine (Family Medicine) degree in 2017 and is accredited as a Family Physician. In addition, she has a Graduate Diploma in Family Practice Dermatology in 2024 from NUS. 

At work, Dr Yong leads the Relationship-based health and social integration (RELATE) programme in NHG Sembawang Polyclinic to help patients with complex health and social care needs. She previously served as an iCare Service leader, Teamlet leader and Asthma Champion. 

In recognition of her contributions to healthcare, Dr Yong received the NHG 10-year Long Service Award in 2025, NHG Polyclinics iCare Champions Star Award in 2024 for service excellence and NHG Polyclinics OurCare Merit Award for Quality Improvement project in 2016. Dr Yong has also received numerous prizes for her research and presentations in 2012.

Dr Yong is a member of the Singapore Medical Association as well as Rheumatology and Orthopaedics Specialty Advisory Group (SAG) in NHG Polyclinics. 

As Clinical Tutor at the Lee Kong Chian School of Medicine and Yong Loo Lin School of Medicine, Dr Yong dedicates her time to train future doctors.
