Assistant Professor Sabrina Wong is Deputy Director of the Clinical Research Unit at NHG Polyclinics, and the Clinical Director of Primary Care and Family Medicine at the Lee Kong Chian School of Medicine. She is also a Senior Consultant Family Physician and a Principal Clinician Scientist. She graduated from the National University of Singapore with an MBBS degree in 2001 and subsequently completed the M.Med (Family Medicine) and Fellowship of the College of Family Physicians. In 2022, she completed a PhD (Medicine) at the Lee Kong Chian School of Medicine focusing on young adults with type 2 diabetes.
To further enhance her expertise, Asst Prof Sabrina Wong was awarded the Human Manpower Development Award in 2023, which allowed her to complete an attachment at the Nuffield Department of Primary Care Health Sciences, University of Oxford. She received the NMRC HPHSR Clinician Scientist Award in 2024, and the NMRC Population Health Grant (Open) in 2025 for the development and evaluation of a digital health coach for weight management in type 2 diabetes.
As a distinguished expert, Asst Prof Sabrina Wong is a Fellow of the College of Fellow of the Academy of Medicine, Singapore. She is the Chief Examiner of the Family Medicine Examination Committee and a part-time consultant with the Ministry of Health.
She previously served as Deputy Director in Clinical Services, where she was instrumental in implementing the Teamlet care model in NHG Polyclinics, which received the Excellence Champion Medal at the National Healthcare Innovation and Productivity Medals 2019. In 2021, she led a team in implementing tele-consultation services for the care of chronic conditions and received the Care Redesign Best Practice Medal (National Healthcare Innovation and Productivity Medals).
Asst Prof Sabrina Wong is co-lead of the Research sub-committee under their NHG Diabetes and Metabolic Steering Committee, and co-lead of the education workgroup. Her team has developed health coaching interventions (PACT) and group education programs (GEM) for people with type 2 diabetes, which has received the Gold NHG Team Recognition Award in 2023 and the NHG Innovation Award (Gold) in 2024. These recognitions reflect her dedication to advancing medical excellence and improving patient outcomes.
Currently, she leads multi-disciplinary teams of clinicians and researchers, focusing on development and implementation of innovative and precision approaches to empower people with chronic medical conditions.
