Prof Chong Phui-Nah is Senior Advisor, NHG Health; Family Physician, Senior Consultant at NHG Polyclinics. She graduated from National University of Singapore with an MBBS degree in 1991 and obtained Master of Medicine (MMED) in Family Medicine degree in 1997. She was admitted as Fellow of College of Family Physicians, Singapore in 2002 and member of Chapter of Family Medicine, Academy of Medicine (Singapore) in 2015.
As Senior Advisor of NHG Health, she provides strategic guidance and mentorship; co-chairs NHG Women’s & Children’s Health Steering Committee for Central-North population health. She is also Clinical Director for the new Asian Centre for Health Behavioural Insights & Interventions. Beyond clinical practice, Prof Chong is a dedicated educator and mentor. She serves as Professor, Clinical Practice at Lee Kong Chian School of Medicine, NTU and contributes actively to training future Primary Care clinicians.
Prof Chong's expertise is widely recognised. She was appointed Senior Fellow, Future Primary Care in MOH Office for Healthcare Transformation (MOHT) in 2023 and has been involved in Healthier SG-related work to strengthen Primary Care. She also serves actively on several national committees, including the SMC Continuing Medical Education Co-ordinating Committee (CMECC), Joint Committee of Family Medicine Singapore (JCFMS), and the Singapore Medical Council (SMC)'s Complaints Panel. She co-chairs MOH’s Open Access Oesophago-Gastroduodenoscopy Workgroup since 2014. She was also Chairperson of MOH Family Medicine Residency Advisory Committee (2016-2019); Co-chairperson for MOH Polyclinic Subcommittee for Service Quality Component (2013-2023).
Prof Chong served as Chief Executive Officer at NHG Polyclinics and Primary Care (2014-2023); Senior Director of Primary Care Academy and Family Medicine Development (2007-2014). Her significant contributions include initiating NHG Polyclinics’ Primary Care transformation in 2015, implementing the Patient Empanelment Teamlet Care Model, which promotes continuity of care of patients with chronic diseases. To raise the standard of Primary Care in Singapore, she established Primary Care Academy (PCA) in 2007 to enhance interprofessional education and skills training. To further strengthen Family Medicine training for undergraduates and postgraduates including NHG Family Medicine Residency, Prof Chong set up the Family Medicine Development department in 2008. She established NHG Polyclinics’ Research Committee in 2001 to inculcate research culture over the years. She was also instrumental with NHG Polyclinics’ GP collaboration since 2008, including developing Family Medicine Clinics (2014-2017) and Central-North Primary Care Network (2018-2023).
In her leadership development journey, Prof Chong was recipient of Ministry of Health's Health Management Development Programme (HMDP) which supported her study attachment in respiratory medicine at London's Charing Cross Hospital and Oxford's John Radcliffe & Churchill Hospitals in 2003. She attended the Executive Development Programme at The Wharton School, University of Pennsylvania in 2009. In 2012, she was awarded the National Healthcare Group Scholarship to pursue an Executive MBA at Chicago Booth School of Business, which she completed in 2014.
Her dedication to advancing medical excellence and improving patient outcomes through innovations were recognised with prestigious accolades such as Public Administration Medal (PPA-Silver) National Day Awards in 2023; Public Administration Medal (PPA-Bronze) and National Day Awards in 2017; Public Administration Medal (Silver) National Day Awards (COVID-19) in 2022; Long Service Award, National Day Awards in 2018; NHG Distinguished Award in 2024; NHG Medallion (Silver) Award in 2023; NHG Recognition Award (Outstanding Citizenship) in 2012; NHG Long Service Award in 2024 and 2012 amongst others.
