Community Health Teams (CHTs) are multi-disciplinary teams comprising doctors, nurses, allied health professionals, pharmacists, health coaches and administrators to provide holistic care for our patients and residents in Central and North regions of Singapore. Our services range from promoting general wellness, case management to stabilisation of care needs.

At NHG, our community nurses and health coaches aim to support and empower you to stay well in the community. Our Community Health Posts (CHP) are located in convenient spaces, offering care services in your neighbourhood.

Advance Care Planning (ACP) is the process of planning for your healthcare and personal care needs. Should you fall too ill to communicate or make treatment decisions, your Advance Care Plan helps your medical team and chosen spokesperson to make decisions that are in your best interests.

We offer ACP Facilitation as a community service, where our community nurses or trained facilitators guide you through meaningful conversations about your values, care preferences, and treatment choices, ensuring your wishes are respected in times of serious illness or medical crisis.

Hospital-to-Home

Hospital-to-Home (H2H) programme helps patients experience a safe and timely transition from the hospital back to their homes. This program offers multidisciplinary care support by the Community Health Team (CHT), tailored to meet the personalized needs of each patient.

If you are a patient in any of the NHG public health institutions and would like to be referred to our programme, please speak with your attending doctor.

EatWise SG (Community-Based Nutrition Care)

EatWise SG is a community-based initiative that brings nutrition care closer to you and beyond the hospitals. Whether you are managing a chronic condition, striving for better nutrition or simply looking to improve your overall well-being, EatWise SG is here to support you.

Depending on your nutritional needs, EatWise SG makes it easy for you to get nutrition care and improve your health:

Access to nutritional programmes: Created collaboratively with Dietitians to promote good health
Expert care: Work with care specialists such as doctors, nurses, dietitians to address your nutrition needs and provide trusted advice
Tailored nutritional advice: Receive support for issues like poor nutrition, malnutrition or managing conditions like diabetes or frailty.

The NHG Assessment & Shared Care Team (ASCAT) is a multi-disciplinary psychiatric team which provides assessment, treatment, and holistic care for clients with moderate to severe mental health conditions. Anchored by four NHG tertiary institutions (Institute of Mental Health, Tan Tock Seng Hospital, Khoo Teck Puat Hospital, and Woodlands Health), NHG ASCAT services offer localised support to community mental health providers and residents within Central-North Singapore.
