The Diabetes Remission (REMI-D) Roadmap – Practical steps for primary care teams
Non-pharmacological approaches to type 2 diabetes remission, in particular very-low-calorie-diet (VLCD) have gained prominence following the landmark DiRECT clinical trial. There is a need for team-based training for clinicians to incorporate VLCD-based programmes as part of routine service delivery to ensure effectiveness and patient safety.

Learning Outcomes
  1. Describe the evidence base for remission of type 2 diabetes remission, including key findings from the DiRECT study
  2. Explain the physiological mechanisms of remission (e.g. ectopic fat reduction, personal fat threshold)
  3. Describe the eligibility criteria and contraindications for very-low-calorie-diet (VLCD)- based remission programmes.
  4. Outline the role of each member of the multidisciplinary team (MDT) in delivering and supporting a dietitian-led remission program.
  5. Apply principles of safe medication deprescribing and monitoring during VLCD implementation according to the protocols provided
  6. Analyse case vignettes to develop safe, patient-centred plans for diabetes remission care
  7. Appreciate the importance of collaborative practice in supporting sustained diabetes remission
  8. Develop and adapt local workflows to integrate remission programmes into teamlet based care
