This course is designed to provide participants with the necessary knowledge and skills to excel in participants role within the primary care setting in Singapore.

Learning Outcomes
  1. Identify key principles and explain the importance of person-centred communication.
  2. Utilize effective communication strategies to promote preventive screening to potential clients in the team-let.
  3. Analyse the specific preventive screening needs of potential clients
  4. Develop strategies to address barriers to preventive screening uptake
  5. Assess fall risk based on a general screening template.
  6. Demonstrate the ability to assist the Care Manager in basic patient education on chronic diseases.
  7. Analyse the role of home blood glucose, blood pressure, and weight measurements in managing chronic diseases.
  8. Evaluate the effectiveness of teaching methods for educating individuals with self-measurement techniques.
  9. Assess the understanding and proficiency of individuals with chronic diseases in performing home measurements.
  10. Evaluate the impact of education and the effectiveness of Health-Hub on self-management behaviours and health outcomes.
  11. Utilize the clinical documentation system to accurately record patient interactions, assessments, and interventions.
  12. Use of basic medical terms - English Chinese translation
  13. Understand mental health screening : Know how to use PHQ-9 and GAD-7 to perform mental health screening accurately
  14. Know how to interpret PHQ-9 and GAD-7 scores and know the management steps after performing PHQ-7 and GAD-9 as a CC brviated mental test
