If you do not wish to have photo, video and/or audio recordings of you being taken during the event, please inform the photographers and/or videographers present. 

By attending our events and/or permitting any such photo, video and/or audio recordings to be taken, you consent and grant NHG Polyclinics, its affiliates, agents and service providers the right to collect, use, store, edit, alter, copy, exhibit or otherwise process your likeness, image and/or voice in any materials published and/or distributed for event publicity and/or other related purposes (the “Uses”). 

The consent and authorisation granted herein for the Uses extends to materials produced in all languages, media and formats (including print, electronic and/or social media channels), and shall continue indefinitely. You acknowledge that unless otherwise expressly agreed, you shall have no proprietary claim over any of the materials and there shall be no compensation for your participation in the event and/or the Uses. 

If you wish to withdraw the above consent, please write to the Data Protection Officer at nhgp.feedback@nhghealth.com.sg, following the receipt of which we will take steps to cease the Uses within a reasonable time period. Please note that such withdrawal shall not affect the rights granted to NHG Polyclinics for the Uses prior to receipt of such withdrawal. 

NHG Polyclinics shall not be liable for any and all claims, demands, costs and/or liability arising out of or in connection with the Uses.
